Case Management

Case Management Services are available to people who have serious and persistent mental illness and/or substance use issues and/or mental retardation.

Case Managers fulfull a commitment which will:

  • provide assessment of your needs and your current situation. This is done by gathering information from you and from your family.
  • link with community resources by informing clients what resources are available.
  • assist clients complete various applications for entitlements such as food stamps, Medicaid, General Relief, or Social Security benefits.
  • monitor the progress a person is making with regard to keeping mental illness symptoms and/or substance use problems controlled and also monitor how well the client is doing in getting and keeping community resources.
  • collaborate (talk with and involve) other professionals and family members or significant others about progress in treatment.
  • coordinate care among all the various resources within the Colonial Services Board and within the community to insure non-duplication of services.
  • offer supportive therapy that support efforts toward positive changes, as well as problem solving to find solutions to issues.

Our Case Management Services strive to provide the client with choices, education and goals. Clients obtain a sense of pride as they expand their friendships, integrate with the community and enjoy the successes related to a new job.

Counseling Services

Individual, family, couple and group clinical treatment is easily accessible and affordable.

  • Our sophisticated Central Access system allows appointments to be scheduled immediately.
  • We are electronically connected with all the major public and private health insurance providers.

Skilled and licensed psychiatrists, clinical psychologists, clinical social workers and professional counselors provide excellent assessments and a varied complement of therapies to assist clients in solving their problems and improving their lives.

Day Support

Our aim is to provide clients the opportunity to develop daily living skills while enhancing the individuals' potential and strengths.

Day support programs operate on a variety of schedules including weekends and evenings.

Services include:

  • People's Place - psychosocial rehabilitation (clubhouse) programs for persons who have serious and persistent mental illness or coexisting substance use problems. Services are provided for two or more consecutive hours a day
  • Individualized planned activities, supports, training, and supervision to improve functioning and maintain optimal level of functioning.
  • Self care and hygiene skill building
  • Task Learning
  • Community Resource Utilization
  • Enviromental and Behavioral Skill Building
  • Center-Based and Community-Based Programs
Employment & Training

Employment and Training Services provide employment supports for program members who work in private sector jobs within the community.

Colonial Workshop - Sheltered employment program providing social, personal and work related skills for individuals who are not ready or unable to enter into competitive employment.

The array of services include:

  • Identification of employment opportunities and resources in the local job market
  • Development of realistic employment goals
  • Establishment of services plans to achieve employment outcomes
  • Identification of resources to achieve and maintain employment
  • Coordination of and referral to employment-related services
  • Paid employment opportunities
  • Supervised employment

Residential Services

Our goal at the Colonial Services Board is to maximize our client's ability to live a quality life in the community through supervised residential and supportive programs.

We offer a full array of individualized residential support for our citizens that include fully supervised apartments that build independent living skills.

These services are offered 24 hours per day, 365 days per year.

Services include:

  • Transitional Living Apartment Program (TLP)
  • Transitional Living Arrangement Program (TLA)
  • MH Support Services Program
  • Independent living skills instruction
  • Congregate living services including overnight supervision
  • Moderately supervised residential support
