Case Management Services are available to people who have serious and persistent mental illness and/or substance use issues and/or mental retardation.
Case Managers fulfull a commitment which will:
- provide assessment of your needs and your current situation. This is done by gathering information from you and from your family.
- link with community resources by informing clients what resources are available.
- assist clients complete various applications for entitlements such as food stamps, Medicaid, General Relief, or Social Security benefits.
- monitor the progress a person is making with regard to keeping mental illness symptoms and/or substance use problems controlled and also monitor how well the client is doing in getting and keeping community resources.
- collaborate (talk with and involve) other professionals and family members or significant others about progress in treatment.
- coordinate care among all the various resources within the Colonial Services Board and within the community to insure non-duplication of services.
- offer supportive therapy that support efforts toward positive changes, as well as problem solving to find solutions to issues.
